ALL APPLICANTS MUST SUMBIT:
- Completed Intake Form
- Copy of Status Card (front & back)
- Copy of Dependent(s) ID
- Resume
ADDITIONAL SUPPORTING DOCUMENTATION:
- Letter of Acceptance from Training Institution
- Job Offer Letter or Employment Contract
- Proof of Program Costs (e.g. tuition, books, parking pass, etc.)

Submitting an application doesn’t guarantee funding. Requests are reviewed based on eligibility and available funds.

Please complete all sections of the intake form. A new form is needed for each request.

Missing or incorrect information may delay your application.

Please allow up to 2–3 weeks for review.

Submit your completed form and documents by email or in person.
