ALL APPLICANTS MUST SUMBIT:

  • Completed Intake Form
  • Copy of Status Card (front & back)
  • Copy of Dependent(s) ID
  • Resume

ADDITIONAL SUPPORTING DOCUMENTATION:

  • Letter of Acceptance from Training Institution
  • Job Offer Letter or Employment Contract
  • Proof of Program Costs (e.g. tuition, books, parking pass, etc.)

IMPORTANT INFORMATION

  • Submitting an application doesn’t guarantee funding. Requests are reviewed based on eligibility and available funds.

  • Please complete all sections of the intake form. A new form is needed for each request.

  • Missing or incorrect information may delay your application.

  • You can fill out the form electronically or by hand.

  • Please allow up to 2–3 weeks for review.

  • Submit your completed form and documents by email or in person.

  • Make sure your contact information is current and easy to read.

  • If a question doesn’t apply, write “N/A.”

  • Ensure all documents are clear and easy to read (scan or photo).
